Background Check Disclosure
CulinaryFleet (“Company”) may obtain information about you from a third-party consumer reporting agency for background screening purposes. The information obtained may include criminal history records, sex offender registry information, motor vehicle records, educational and employment history verifications, professional licenses and credentials, address history, credit history (where permissible), and other information relevant to determining eligibility for contracting with CulinaryFleet.
You may also be the subject of an “investigative consumer report,” defined as a report that may include information as to your character, general reputation, personal characteristics, and mode of living. This information may be obtained through personal interviews with your prior employers, associates, and other persons who may have knowledge of you.
The report will be obtained for purposes of evaluating your eligibility to contract with CulinaryFleet, and for ongoing evaluation throughout the duration of your relationship with the Company. All information obtained will be in compliance with the federal Fair Credit Reporting Act (FCRA), applicable state laws, and other applicable laws.
You have the right, upon written request, to be informed whether a consumer report was requested and to request a copy of the report. You also have the right to dispute any information contained in the report by contacting the consumer reporting agency.

Authorization
I hereby authorize CulinaryFleet and its authorized agents to obtain consumer reports and/or investigative consumer reports about me for initial onboarding, periodic reassessment, retention decisions, and for any other lawful employment or contract-related purposes. I understand that the information obtained may include, but is not limited to, criminal background checks, public records, driving records, and verification of personal information.
This authorization shall remain in effect for the duration of my engagement with CulinaryFleet unless revoked by me in writing.
State-Specific Disclosures
California: You have the right to request whether an investigative consumer report has been requested and to request a copy of your report under California Civil Code Section 1786.22.
Minnesota: You have the right to request a copy of the consumer report obtained.
Oklahoma: Please check this box if you wish to receive a copy of your consumer report.
New York: You have the right to request whether a consumer report has been requested and to inspect and receive a copy of the report by contacting the consumer reporting agency.
Washington State: You have the right to request a copy of your report by contacting the consumer reporting agency directly.
